Family Photography Questionnaire

I would like to get to know you a little better before we meet for our photography session. Please fill out this questionnaire so I can do my best to fit the session to your needs.

Do you have a location in mind you would like for me to consider? Or do you need help coming up with ideas?

List everyone who is participating and their relation to you. Please also list the ages of any children.

STEEPLY DISCOUNTED PRICES to help me build my portfolio! Come dressed and ready to work together to get some great photos. Bring any props you might want to use. Working with me will be a breeze and you'll get some keepsakes that will last a lifetime. One athlete per session. Fee for distances greater than 30 miles. Prices will go up immediately January 1, 2024.
